Care Worker and Home Carers - 5 Positions available
Provide holistic person-cantered support to clients; act as champions for clients’ safety, respect, choice, and dignity as benchmark standards for all good clinical and care governance practice in line with Carers Code of Conduct, CQC and NICE Guidelines.
Professionally support clients with holistic care delivery during any span of duty, ensuring that a safe, effective, and efficient care service is delivered and maintained to meet the needs of vulnerable clients receiving complex care.
Support clients with social and physical activities; booking and attending appointments, helping with personal care such as showering and
Monitoring individuals’ conditions, and possibly helping with medication following training and competency assessments.
Ensure clear and accurate records and communication is maintained always relating to clinical and care delivery.
Liaise with Multi-health professionals, relevant stakeholders, and Families with consent to ensure that all clients receive the holistic medical, clinical, social, and recreational support they require and desire.
